In this edition we present two cases investigated by the coroner’s court because of medication errors a common and dangerous aspect of aged care.


In one case the resident did not receive their regular anticonvulsant medication which lead to seizures and aspiration. In the other case a resident was inadvertently administered medication prescribed for another resident.


Our expert commentary offers an approach on how to apply systems thinking to medication safety explaining why urging staff to ‘be more careful’ is not enough.
