All aged care homes must continue to provide the care for residents irrespective of whether directly or indirectly affected by the COVID-19 pandemic. In this edition we present a case where a misunderstanding between medical and nursing staff led to preventable harm from a caustic agent to a much loved resident with dementia. All the staff involved were well intentioned and working in good faith.


Three expert commentaries address the underlying systems issues of human factors and the importance of building our non-technical skills as well as reinforcing the importance of pain management especially in persons with dementia and the merits and hazards of off-label therapy and medication management..
